Sick (fit) notes
If you’ve been off sick for more than 7 days
To request a sick note:
- Fill out a SystmConnect request form
- Phone or visit the surgery
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
However, some employers insist on a Doctor’s note regardless. A Doctors note under these circumstances involves a charge of £10.

Requesting an additional Sick/Fit Note
If you have already had a Sick Note (Fit Note) for this illness your Doctor may not need to see you to issue an additional Sick Note.
Please make a Fit Note request via SystmConnect or send us a SystmOnline/NHSapp message with the dates you need for a continuation sick note.
We will contact you to let you know when you can collect your Sick / Fit Note or we may contact you to arrange an appointment.
As we are able to send sick notes via text or email, please ask the Reception team to confirm your contact information on your records in order to avoid any delays. Alternatively, you can change these details via SystmOnline, Airmid or the NHSapp.
Did you know?
Hospitals can also issue fit notes. If you have just had surgery or are receiving treatment which causes you to be too sick to work, etc. You can request the fit note whilst in Hospital.
